If you have selected “Yes” to at least one of the questions in the Impact Assessment section on the Activity Details screen, you must complete an “Impact Assessment Form” (Appendix A) and upload it to the Impact Assessment page. If none of the situations apply to your proposed research activities, then Appendix A is not needed.
